COPPA Compliance

Your child’s instruction includes access to, and the use of, The Collective Music Theory Application, a software application published by Collective Music Theory, LLC. The software application and website provide music theory instruction, testing, and performance practice exercises for your child. To utilize The Collective Music Theory Application, your child will be required to create a login ID that is dedicated to your child. As such Collective Music Theory, LLC is required to provide this notice to you and obtain your affirmative consent regarding its collection, use, and disclosure of your child’s personal information.

(i)             Collective Music Theory, LLC (“CMT”) will collect your child’s name, email address, a photo of your child (if submitted) and selected username (collectively “Information”) when your child registers for an account or updates their account with the Collective Music Theory Application.

(ii)           Your consent is required for the collection, use, and disclosure of the Information by Collective Music Theory, LLC. Collective Music Theory, LLC will not collect, use, or disclose any of the Information without your express consent. However, failure to provide consent will affect your child’s ability to use the Collective Music Theory Application.

(iii)         Other than the Information, there is no other personal information of you or your child that Collective Music Theory, LLC intends to collect. Collective Music Theory, LLC will disclose the Information collected to your child’s school.

(iv)          Collective Music Theory, LLC’s privacy policy detailing how the Information will be collected, used, and disclosed can be reviewed at the following link: https://www.collectivemusictheory.com/policies

(v)           Your signature below is your verifiable consent that Collective Music Theory, LLC can collect, use, and disclose the Information as set forth herein and in its privacy policy. You may also provide verifiable consent by sending an email to support@collectivemusictheory.com stating your name, your child’s name, and that you have received this notice and consent to the collection, use, and disclosure of the Information as set forth herein and in the above privacy policy.

(vi)          By signing, you acknowledge the embedding of YouTube videos in the application, and that YouTube does not verify that videos are specifically COPPA or New York State compliant.

(vii)        You may deactivate the account at any time by clicking “Deactivate Account” in the “Update Account Info” page, accessible on the Profile. Doing so will immediately delete all personal information from Collective Music Theory, LLC’s databases. Permanent deletion from all backup data and platforms will occur within 7 to 150 days.

(viii)      If applicable, if you do not provide consent within a reasonable time from the date this notice was sent, Collective Music Theory, LLC will delete your online contact information from its records.
